CSF Neuroscape Study: Step-by-Step Guide for Participants

Welcome! This guide will help you get started with the UCSF Neuroscape study. Follow each step carefully, and you’ll be ready to participate in our research.

Part 1: Create Your Nexus Account

  1. Open your web browser and go to nexus.ucsf.edu
  2. Click Get Started
  3. Click Create Account
  4. Enter your email address, age, first and last name, and choose a password
  5. Read and accept the registration statement
  6. Go to your email inbox and verify your account
  7. Once verified, it will say Registration Complete → click Continue

Part 2: Log In & Explore Your Profile

  1. You will return to the home page → click Login
  2. Log in using your email and password
  3. Click Get Started → you’ll see a short tutorial
  4. Click on your name/profile to:
  • View or edit your information
  • See previous studies
  • Enroll in new studies
  1. Click Join a Study to enroll in any study you are eligible for
  2. Enter a study code if you have one; otherwise, continue without a code
  3. Enroll in the Eligibility Study – a 20-minute questionnaire to see which studies you may qualify for
  4. Click Neuroscape Studies Open Enrollment to see all currently recruiting studies
  5. Check your To-Do List (click Tasks in the top left) – here you will find surveys, activities, and interactive games

Part 3: Eligibility Study

  1. Click Join Study → you will return to your dashboard
  2. Under Eligibility Study, click Enroll → complete this general eligibility survey
  • This survey helps coordinators match you with suitable studies
  • Takes 30 minutes to 2 hours
  • You may be contacted later for other studies even if none are currently suitable
  1. Click Join Eligibility Study
  2. Read the Consent Eligibility Screening → click the arrow at the bottom to continue
  3. Answer a few questions about the consent form correctly (you have 3 attempts)
  4. Start the Eligibility Screening Questionnaire
  • About your age, health, lifestyle, and other factors
  • Takes ~20 minutes
  • If you completed it in the last month, do not repeat it
  • No payment for this questionnaire
  • Ages 60–85 may be asked to complete cognitive assessments afterward

 Tips for Success

  • Make sure your device is charged before starting
  • Use a quiet space for tasks requiring focus
  • Contact your study coordinator if you run into issues

Nexus Registration Instructions (Older)

Nexus is a mobile study platform developed and used by Neuroscape at the University of California, San Francisco. Neuroscape is a translational neuroscience center at UCSF engaged in technology creation and scientific research on brain function.

Enrolling in a Study (Two Ways)

  • Enter a study code (tahda) on the participant landing page
  • Or browse current studies on Nexus:
    • Click a study card to view details and enroll

ACE! (45–60 minutes total)

Note: ACE must be played on a desktop or laptop computer.

  1. Open the first ACE Explorer task → click the Web link
  2. On the ACE login page:
    • Click User account
    • Log in with the same credentials as Nexus
  3. Play instructions:
    • Put browser in full-screen mode
    • Select language preference
    • Enter your name as username (if asked)
    • Click Start
  4. Complete all ACE modules (~30 minutes)
  5. After completing and signing out:
    • Return to Nexus (https://nexus.ucsf.edu) to finish remaining tasks
  6. You will be asked to play another set of ACE modules (~additional 30 minutes)

UCSF TAH-DA Samsung Participant Guide

Weekly Tasks

  • Keep your watch and tablet charged and powered on.
  • Use the tablet to check the Nexus app for new tasks.
  • Take weekly watch measurements: ECG, Body Composition, and SpO2.
    • You may take these more often than once per week if you wish.
  • Wear the watch during the day and during the night.
  • Ensure the watch has at least 50% battery before going to sleep.

Tips

  • Set a charging routine and an alarm to remind yourself to take devices off the charger.
  • Keep the tablet charging in a location with good connection.

Study Tasks and Tips

  • Wear the Watch: Every day and night as much as possible throughout the one-year study.
  • Weekly Watch Measurements: Once per week measure ECG, Body Composition, and SpO2.
    • Tip: Build this into your routine (same time each week).
    • Tip: Put it on your calendar or use reminder notes.
  • Use the Nexus App: Check and complete study tasks.
  • Charge Watch & Tablet:
    • Tip: Set an alarm during charging to put it back on.
    • Tip: Charge during a daily routine like cooking dinner or showering.
    • Tip: Keep charger in a frequent location (living room/bedroom).
    • Tip: Avoid keeping charger in a drawer or a low-signal room.
  • Water Resistance: Devices are water-resistant, not waterproof (see Water Resistance sections).

Reminders (Changing Reminder Time/Day)

If default reminder time/day doesn’t work, change it on the watch:

  1. Open the study app → swipe up → tap Reminders
  2. Toggle a reminder type on/off, or tap the reminder to edit (Body composition, SpO2, Sleep, ECG)
  3. Use the wheel to pick reminder time
  4. Select/unselect days → tap Done
    • For Body Composition: only one day can be selected
    • For daily ECG reminders: select all days

Syncing Study Data

  • The watch automatically sends study data while charging.
  • If data hasn’t been sent in 3 days, you’ll get an email and a watch reminder.
  • Data is synced only while charging (look for the data-syncing icon).
  • You’ll continue to receive reminders until data is synced.

Samsung Galaxy Tablet Basics

  • Keep tablet turned on and charged.
  • Use tablet when Nexus app notifies you.
  • Use in a location with good cellular service or Wi-Fi.
  • Buttons/ports:
    • Power Button: on/off
    • Volume Button: adjust volume
    • Charging Port: plug in charger

Tablet Charging

  1. Plug charging block into an outlet
  2. Plug charger into tablet
  3. Confirm charging indicator on screen
    • Battery icon indicates charging
    • “100” indicates fully charged
  4. Keep tablet charged/powered on in an area with cellular reception

Touch Screen Notes

  • Screen protectors/accessories may affect touch functionality
  • Gloves, dirty hands, sharp objects, and humid/wet conditions may cause touch issues
  • Restart device to clear temporary software problems

Connecting the Tablet to Wi-Fi

  1. Tap Settings icon
  2. Tap Connections
  3. Tap Wi-Fi
  4. Select network
  5. Enter password (if needed) → tap Connect

Note: For many networks, the name is printed on the router.

Write down your Wi-Fi details:

  • Network name: ______________________________

Watch Overview

  • Charge watch daily (~1 hour) or every other day (~2 hours)
  • Wear snugly on the left wrist during daily activities and sleep
  • Take study measurements weekly by tapping the blue/white heart symbol on the watch face

Activating Your Account (Watch)

  1. Tap Activate
  2. Wait for activation progress
  3. Tap Okay when complete

ECG Spot Check App Troubleshooting

Common issues:

  1. Dry skin: moisturize wrist/finger or “fog” finger with breath
  2. Excessive motion: stay still; support limbs
  3. Poor electrode contact: ensure snug fit; skin not too dry; try moisture
  4. Finger removed too soon: keep finger on button for full 30 seconds

Turning the Watch On/Off

Power On: Press and hold both buttons (Home + Back) until it vibrates and “Samsung Galaxy” screen appears.
Power Off: Press and hold both buttons → tap Power off, or open quick panel and tap Power off icon.

Watch Charging

  • Plug block into outlet
  • Place watch centered on wireless charger
  • Watch needs to remain on while charging to transmit data
  • Charge before battery drops below 15% (watch won’t collect data below that)

How to Take ECG Spot Checks

  1. Sit comfortably, arms supported, feet flat, minimal motion
  2. Tap Start on ECG reminder (or open study app → ECG)
  3. Rest right finger on top right button (no need to press hard)
  4. Stay still for 30 seconds
  5. When done, tap Home

Taking Weekly Watch Measurements

Once per week record:

  • ECG
  • Body Composition
  • SpO2

Open study app from watch home screen and choose measurement type.

ECG Notes

  • ECG spot check takes 30 seconds
  • ECG app does not look for signs of heart attack
  • Do not use ECG app if you experience:
    • Shortness of breath
    • Chest tightness or pain
    • Fainting
    • What you think might be a heart attack

Common Watch Questions

How do I avoid skin irritation?

Remove watch if wearing for long periods, sweating, or swimming. If irritation occurs, stop wearing and let skin heal. Contact your doctor if it doesn’t improve, and inform study team.

Can I wear the watch when exercising?

Yes.

Is the watch waterproof?

The watch is water resistant, It is recommended for study purposes that the watch is removed when going in water. However, The watch can't be submerge for 30 minutes in shallow waters.

Watch Water Resistance Tips

  • Dry thoroughly with a clean, soft cloth when wet
  • Do not expose to high-pressure water
  • Avoid salt water; if used, rinse with fresh water and dry
  • Impacts/drops may reduce resistance
  • Avoid sudden temperature changes and high-velocity activities
  • Not intended for scuba diving
  • Can stay under 5 feet of water up to 30 minutes

How often should I wear the watch?

Wear as often as possible, including during sleep. Aim for:

  • Daytime: 4–5 days per week
  • Nighttime: at least 1 night per week
    Place on charger when not wearing.

Should I leave the watch on while charging?

No. Take the watch off to charge it when needed. A helpful tip is to set a reminder on your phone so you do not forget to put the watch back on after charging.

What if the watch won’t turn on?

Battery may be fully drained. Place on charger and wait for charging status to appear; it may take a few minutes. Wait until 5%+ before pressing Home to power on.

What if I’m having problems with the watch?

Restart the watch (off/on). To restart the watch hold down the power button for 7 seconds. If not resolved, contact study team in Nexus app on tablet.

Cleaning the Watch and Band

Wipe sensor area and band with a soft, lint-free cloth. For stubborn grime, lightly dampen cloth with mild soap solution or 70% isopropyl alcohol. Dry thoroughly. Avoid excessive cleaning solution or scrubbing.

How to Wear the Watch (Fit Guide)

Wear on left wrist, comfortably snug.

Incorrect:

  • Too loose: two fingers fit between watch and wrist
  • Too tight: one finger doesn’t fit

Correct:

  • One finger fits between watch and wrist
  • Two fingers do not fit
  • Watch face lays flat and is not touching wrist bone

Consent to Participate in a Research Study (TAH-DA Study: 2025.06.17)

Study Time Commitment

Total passive time: 50 weeks

  • Wear smartwatch daily and nightly for 50 weeks (biometrics such as heart rate, skin temperature, step count)

Total active time: 37 hours over 50 weeks, across phases:

  1. Baseline: 5 hours over 10 weeks
  2. Intervention: 18 hours over 12 weeks (30 min, 3 days/week)
  3. Waiting Period: 4 hours over 8 weeks
  4. Booster: 6 hours over 12 weeks (30 min, 1 day/week)
  5. Follow-Up: 4 hours over 8 weeks

All activities are remote.

Risks / Discomforts

Minimal risks. Most common:

  • Questionnaires: boredom, discomfort answering questions
  • Assessments/training: mental fatigue, boredom
  • Smartwatch: some functions can interfere with cardiac implantable electronic devices

Data Collection & Sharing (Summary)

Data includes identification info, demographics, health, cognitive assessments, and smartwatch biometrics (heart rate, SpO2, PPG, actigraphy, skin temperature, body composition). May be shared with representatives of UC, Samsung Research America, Kajeet, QuantActions, Help Squad, Didit, and other researchers. Samsung/QuantActions/other researchers receive de-identified data; Kajeet needs name/address for shipping; Help Squad provides technical support and can work via anonymized Study ID; Didit performs ID verification via camera comparing face with government ID.

Payment / Devices

  • No payment for completing the full study
  • If you complete all procedures, you may keep the smartwatch and tablet (as personal property)

Prorated amount to be received by participant if not completing the full study:

  • $0 for baseline outcomes + up to five training sessions OR wearing watch <50% of baseline phase
  • $50 Amazon e-gift card for 6+ training sessions and watch worn at least 50% but not all tasks complete
  • $100 Amazon e-gift card for completing through Waiting Period and watch worn at least 50% but not all tasks complete

If not completing, devices will be locked and must be returned for prorated payment (shipping label provided).

Voluntary Participation

You can stop at any time without penalty. Any data already collected remains in study records. PI may remove you if in your best interest, study rules not followed, or study stopped.

Welcome

Welcome to the TAH-DA study! Total study is ~50 weeks.

Baseline (weeks 1–10):

  • ~30 minutes per week (surveys + assessments)
  • Weekly tasks open as a set; complete in any order
  • No need to wait for previous task to mark complete before doing next
  • Recommended: set a weekly time block (flexible timing, but finish each task before taking a break)
  • Each baseline week designed to be ≤ 30 minutes

Intervention (weeks 11–22):

  • ~2 hours per week
  • Includes interventions, wearing Samsung watch, weekly watch measurements, and weekly survey about measurements

MDT — Part 1

View images and answer questions; complete in one sitting. ~30 minutes.

Rhythmicity

Assess rhythmic abilities. ~15 minutes.

ORCA

ORCA (Older Adult Research Characterization App) is a 30-minute digital screener on Samsung tablet.

  • Press Start
  • Select “User Account” and log in with Nexus email/password
  • If not on tablet: go to tablet and open via Nexus app button or open ORCA directly

Expectancy Survey

Asks you to judge how much cognitive improvement might be seen after using assigned digital technologies. “105 minutes” appears in source text.


Intervention Phase (Schedule + App Descriptions)

Interventions (Welcome)

You will use three digital technologies, wear the Samsung watch, and take weekly watch measurements over the next 12 weeks.

Goal: complete ~30 sessions on each technology within 12 weeks.
Limit: at most one session per technology per day.

Play 2 Games per day, 3 days a week during the intervention phase. You can pick what order you play the games and what two games you pair together in a day, By the end of the week regardless of the order you plays the games in, You will have played all three games twice in one week.

Suggested schedule (example): two technologies for ~12 minutes each every other day, with alternate days off.

This version includes a draw pile and follows classic Solitaire rules, while helping researchers measure attention, planning, and strategy.

Let’s walk through it step by step.

Understanding the Layout

When the game starts, you’ll see three main areas:

1️⃣ Tableau Columns (Main Playing Area)

Location: Center of the screen
These are the large vertical columns of cards laid out across the middle.

  • Cards are stacked in columns.
  • Some cards may be face down.
  • This is where most of your moves happen.

You will arrange cards here in descending order (King → Ace) and alternate colors (red on black, black on red).

2️⃣ Foundation Piles

Location: Top left of the screen

These are four empty slots at the top right.

  • Each pile represents one suit (Hearts, Diamonds, Clubs, Spades).
  • You must build each pile in ascending order (Ace → King).
  • Cards in each foundation must stay in the same suit.

Your goal is to move all cards here.

3️⃣ Draw Pile

Location: Top right of the screen

This is the stack of remaining cards that were not dealt into the tableau.

  • Tap the draw pile to reveal a card.
  • You can play the revealed card into the tableau or foundation if there’s a legal move.
  • If you can’t use it immediately, you may need to draw again.

How to Play

Step 1: Build in the Tableau

  • Stack cards in descending order.

  • Only Kings can move into empty columns.

Your Goal

Fill in the grid so that:

  • Each row contains the numbers 1–9 with no repeats.
  • Each column contains the numbers 1–9 with no repeats.
  • Each 3×3 box contains the numbers 1–9 with no repeats.

Every number can appear only once in each row, column, and box.

Step-by-Step Guide

1️⃣ Look at the Starting Numbers

Some numbers are already filled in.
These cannot be changed.

They are your clues.

2️⃣ Pick an Empty Cell

Tap or select an empty square in the grid.

3️⃣ Eliminate Possibilities

Before placing a number, check:

  • Does this number already appear in the same row?

Enrolling in the Study
If You Are New to Nexus (Recommended Method)
1. Open the study enrollment link:
https://nexus.ucsf.edu/register/neuroscape?participant_group=tahda
2. Review the study information and select Join.
3. Create your Nexus account. If you already have a Nexus account, select Log In
instead. Use an email address and password you will remember.
4. Verify your email. Look for an email from 'Nexus Team' titled 'Verify your email.'
Check your Spam/Junk folder if needed. Open the email, select 'Open in Nexus,' then
click 'Complete.'
5. Return to the Nexus website and log in.
6. Select Join Study (during or after the tutorial).
7. Enter the study code: tahda (lowercase only, no spaces or dashes).
Alternative Enrollment Method

2. Select Get Started, then Create an Account.

After Enrolling

5. Additional surveys will become available each week during participation.
Please note: You will not begin using the study smartwatch or tablet immediately. These
devices are introduced a few weeks into participation. Until then, complete study activities

I emailed the TAH-DA study team but have not received a response.

Watch Setup & Navigation

How do I navigate my smartwatch?
 The top side button is the power button.
 The bottom side button is the Home button.
 Press the Home button to return to the watch home screen from any app.
 Press the power button, then swipe up from the bottom of the screen to
unlock the watch.
 Tap the screen to open an app.
 Swipe down from the top of the screen to access settings.
 Scroll through apps by dragging upward on the screen.
 Within an app, scroll or swipe left and right to navigate.
How do I activate my watch?

1. Unlock the watch.
2. Open the DHT Health Monitoring app (heart icon with a light blue circle).
3. Select Activate.
Weekly Watch Tasks

What am I required to do with the watch each week?

Once each week:
1. Open the DHT Health Monitoring app on your watch.
2. Complete all four biometric measurements by following the on-screen
instructions.
3. Complete the Weekly Check-In Survey in Nexus.
Important: The biometric measurements and Weekly Check-In Survey are two
separate tasks. Both must be completed every week.
Do I need to transfer my watch data to Nexus?

No. After completing the biometric measurements in the DHT Health Monitoring
app, your data is automatically transmitted to the study team. No additional
syncing is required.
Can I view my health data over time?

Open the Performance tab within the Nexus Tablet app.
If no information appears, you are likely an early participant. This feature is still
being implemented and will become available in a future update.
How often do I need to wear my watch?

To remain enrolled in the study, participants must wear the watch for more than
one full day and one full night each week, on average.
You are welcome to wear it more often, but this is the minimum required level of
participation.
Watch Troubleshooting

My watch isn’t working properly.
Restart the watch:
1. Hold both side buttons for approximately 7 seconds.
2. Select Restart.
3. Wait for the watch to reboot.
If the issue continues after restarting, contact the Study Coordinator.
I receive a “400 Error” when activating my watch.
This usually means the Study Coordinator has not yet completed activation from
the UCSF side.
Please allow up to 2 days after receiving your watch for activation.
If you continue receiving this error 3 days after delivery, contact the Study
Coordinator.
I receive a “No Network Service” error when activating my watch.
Restart the watch using the instructions above.
If the error continues, contact the Study Coordinator.
My watch battery will not charge or drains unusually fast.
Restart the watch using the instructions above.
If the charging issue continues, contact the Study Coordinator.
Nexus App

I am experiencing a general error in the Nexus app.
Try the following steps:
1. Log out of the Nexus app.
2. Log back in.
3. Restart your tablet if the issue continues:
o Hold the power button for approximately 30 seconds.
o Wait 30 seconds.
o Hold the power button again until the Samsung logo appears.
The due dates in my Nexus web account and tablet do not match.
This is a known issue.
Please follow the due dates shown in the Nexus Tablet app.
Tablet

When do I begin using my tablet?

Complete study activities through the Nexus web browser until you reach the
ACE Explorer – Tablet task.
After that point, complete study tasks using the Nexus Tablet app.
My tablet is experiencing an error.

Restart the tablet:
1. Hold the power button for approximately 30 seconds.
2. Wait 30 seconds.
3. Hold the power button again until the Samsung logo appears.
My tablet says “No Tasks Due,” but I believe I have pending tasks.

1. Log into Nexus on a computer or phone.

Which games should I play during the intervention phase?

Participants should play three days each week.
On each of those days:

 Each game lasts approximately 14 minutes
You may choose any two different games each session.
